After School Registration for the upcoming 2024-25 school year is Open
The ÁùºÏ±¦µä 3rd Base Program utilizes a digital registration form that can be accessed below:
- 2023-24: After-School Registration is open and will continue throughout the school year. During peak times of enrollment, forms could take 5-10 business days to process.
- Registration forms are processed in the order they are received.
- Once your form is processed you will receive further instructions to make a payment and complete the registration process.
- A Waiting List may be applicable if a campus reaches enrollment capacity as designated by HHSC Minimum Standards.
- Status will be communicated via email correspondence after your registration form has been processed.
- All required information must be entered correctly. Missing information will result in a delay of enrollment. Items you will need to submit on the form:
- Local Emergency Contact (someone other than the parent/guardian, HHSC Minimum Standard Requirement)
- Full name, local address, phone number, and relationship to the child
- Local hospital designation
- Child’s doctor information
- Local Emergency Contact (someone other than the parent/guardian, HHSC Minimum Standard Requirement)
